Annual Wellness Visit

There’s no cost to you for your Annual Wellness Visit, a comprehensive annual visit with your primary care physician (PCP). It’s an important way to help maintain or improve your health and well-being.

Your Annual Well Visit Includes:

• $0 copay.*
• A detailed exam and conversation with your PCP about your health.
• Your annual well visit will help you and your doctor create a personalized care plan that’s right for you.
• Topics for discussion at your annual well visit may include the importance of preventive services (such as vaccines), cancer screenings (such as mammograms and colonoscopies), medication adherence, maintaining healthy bones, preventing falls, bladder issues, advanced directives (such as a Health Care Proxy), quitting smoking and more.
  

Schedule an appointment with your primary doctor’s office and be sure to tell them that the appointment is for your Annual Wellness Visit.

Independent Health is a Medicare Advantage organization with a Medicare contract offering HMO, HMO-SNP, HMO-POS and PPO plans. Enrollment in Independent Health depends on contract renewal. Member must use in-network providers to take advantage of this benefit (excluding Independent Health’s Medicare Passport Connect PPO plan). *Benefit varies by plan, Independent Health’s Passport Connect PPO plan will have a copay if you see an Out-of-Network provider.  The Annual Wellness Visit is separate from the Welcome to Medicare Visit, they cannot both be used in the same year. The one-time Welcome to Medicare preventive visit is only within the first 12 months of having Medicare Part B.
