Your Rights as a Member
Upon joining Independent Health, a member contract and/or amendments are sent to you each year. This information outlines the rights, responsibilities and benefits you have as a member.
Requesting Member Personal Information
When Independent Health needs to request personal information from our members, we will contact the member and request that they call Independent Health at the number that is listed on the member’s ID card. Unless the member initiates the contact Independent Health will NOT request personal information (e.g., Social Security number, mother’s maiden name, etc.) through email, U.S. mail, or phone.
