Medical Management

Independent Health members are stratified to optimize resources for the greatest impact. Our member outreach and support are customized to ensure they receive the right care, in the right place, at the right time.

The Independent Health RedShirt Rewards program provides incentives for members just for completing actions that can help them manage their health and wellness.

Each member in their plan is eligible to earn up to $30* in rewards each plan year when they complete healthy activities such as an annual checkup, flu shot** and more. Members can redeem their reward(s) at a wide range of participating partners, including Amazon.com, Nike, Target and more.

Innovation for better care

To address gaps and strains in the health care system, Independent Health is working within the system and with Independent Health members who have chronic conditions to personalize care experiences. By creating greater efficiencies and a better overall experience, we have the opportunity to deliver lower costs and better trends.

Reducing Readmissions

Individuals with multiple chronic conditions, behavioral health issues, or new diagnoses like cancer or diabetes often require significant support to navigate the health care system and avoid hospital readmissions. To address this, Independent Health funded integrated teams of Great Lakes Health System clinicians and support professionals to provide onsite care at hospitals for high-risk patients, focusing on discharge planning and 30-day follow-up care.

  • 56% reduction in commercial readmissions within the first year
  • Buffalo General and Millard Fillmore Suburban now have the lowest readmission rates of any hospital system in the community for Independent Health members.

Behavioral Health

Mental health issues are increasingly affecting employees’ emotional well-being, physical health, relationships and work performance. Total cost for members with a Behavioral Health medical claim is approximately 42% higher than those without.*

The Independent Health Behavioral Health Case Management program collaborates with members, providers, and support systems to ensure individuals receive appropriate care.

  • Independent Health’s Behavioral Health provider network in WNY includes over 1,000 practitioners and approximately 26 facilities and hospitals.
  • Teladoc® Mental Health provider network includes more than 5,400 Mental Health experts including Psychologists, Therapists and Psychiatrists.
