Living Healthy Diabetes Self-Management Program

Diabetes Stick

The Diabetes Self-Management Program is a 6-session workshop designed for people living with type 2 diabetes or people who care for them. Participants learn self-management techniques and skills needed in the day-to-day management of diabetes.

Living Healthy Chronic Disease Self-Management Program

The Chronic Disease Self-Management Program is a 6-session workshop designed for people living with an ongoing or limiting health condition or people who care for them. Participants learn self-management techniques and skills needed in the day-to-day management of any type of ongoing health condition.

Good For the Neighborhood

Good For the Neighborhood logo

Good for the Neighborhood is a free, community-based wellness program that brings health-related training and tools to areas where access to health care is limited. Individuals and families are encouraged to manage and improve their health by attending quarterly health fair style events and workshops.
