Bronze Plans

Plans offered at the bronze level feature the lowest premium along with higher deductibles and/or coinsurance, including Choice Plus Bronze, Prime Access Bronze, iDirect Bronze, NY/PA Bronze and Passport Plan Bronze.

At this level, there are still flexible out-of-area options available as well as our innovative tailored provider network products: Choice Plus Bronze and Prime Access Bronze. Plus, these plans also offer the NY Standard Gym Benefit or the choice of either our nutrition benefit, which offers you money-back rewards, or our easy-to-use personalBest! health and wellness benefit.

Coming Soon for 2015! Choice Plus® will now offer the advantage of an expanded, high-performing physician network – with physicians from The Primary Connection®. Plus, our all new Max® plan offers unique lifestyle benefits to help you live a healthier lifestyle. Learn more
