The Primary Connection
The Primary Connection is an innovative, physician-led initiative that takes a holistic approach to achieve better quality, better access to care and more affordability. This collaborative partnership between physicians empowers them to achieve better coordination of care with specialists and other health care providers, improve the patient care experience and accomplish better outcomes – which will lead to lower costs in the long term.
Catholic Health/Catholic Medical Partners
Because Independent Health, Catholic Health and Catholic Medical Partners share a common vision for providing patient-centric health care, we have joined forces to offer employers new products designed to achieve better health, better care and lower costs through the stronger coordination of patient care.
HEALTHeLINK, one of the first health information exchanges in the country, facilitates the real-time electronic exchange of clinical information, with a patient’s consent, between physicians, hospitals, insurers and other medical professionals to promote collaboration, limit duplication and control health care costs.
As of March 2013, there are 2,627 participating providers, nearly 450,000 participating patients and more than 89 million results (lab, radiology and transcribed reports) recorded. Embracing new technology is just one way Independent Health is helping move our region forward toward better care
and better health.
