The Primary Connection, in partnership with Independent Health, launched a new approach to transform the delivery of health care in Western New York, which uses patient-centered medical home (PCMH) principles as its foundation.
This alliance of primary care physicians is designed to focus on improving patients’ experience of care, enable closer coordination with specialists and other providers, and achieve better outcomes, which will lead to lower health care costs.
More than 25 primary care practices and 180 adult and pediatric primary care physicians have joined the initiative.
